A Webinar with Population Health Management

The Population Heath Management National Programme Series are running a webinar ‘How can ICSs and STPs work better with the third sector to deliver PHM and improve lives?’ 

The webinar will provide the opportunity to hear from the national Population Health Management (PHM) and Voluntary Sector Partnerships teams and from key partners.

 Discover how systems and third sector partners are using PHM approaches to improve lives. There will also be the opportunity ask any questions you may have following each presentation.
